Step 3: Fill out the following forms and upload to My Private Folder
- Transaction Form
- Commission Disbursement Form (CDF)

The FMLS charges a fee to use their service = Sales Price x .0012
Please enter the correct amount on the Commission Disbursement Form
Step 4: Request the broker’s signature on the Commission Disbursement Form in 3 ways
Option 1 – Send an email to [email protected] (please note the file name or folder)
Option 2 – Request through your e-Signature account (DootLoop, DocuSign, etc.)
Option 3 – Use the “Send for Signature” feature within My Private Folder
Once the CDF is signed by the broker, you will need to send it to the person who will disburse the commission.
Please don’t wait until the last minute to request the signature, we recommend 2 business days. DO NOT EMAIL US ANY DOCUMENTS! Upload everything to My Private Folder.
